Pathway: elapsed <= 270 THROMBOLYSIS_WINDOW; else elapsed <= 1440 with suspicion THROMBECTOMY_EVAL; else OUTSIDE_WINDOW. Destination COMPREHENSIVE for any large-vessel suspicion, else NEAREST. Return {score, lvo, elapsed_min, pathway, destination}.
