Input {weight_kg, aptt (integer seconds), rate (units/kg/h)}. Toy nomogram with dosing weight capped at 120 kg: aptt < 35 bolus 80 u/kg and rate +4; 35-45 bolus 40 u/kg and rate +2; 46-70 no change; 71-90 rate -2; > 90 hold 60 minutes and rate -3. Rate is never below 0. Bolus units = bolus u/kg * weight rounded half-up to the nearest 100 units. Return {bolus_units, hold_min, rate_u_kg_h, rate_u_h (half-up integer of rate*weight)}.

Nomogram-driven infusion adjustments combine band lookup, weight caps and pump rounding; each is a separate failure point.
