Input q (per 100000 from the issue age), n, i (per mille), benefit (cents), timing eoy|immediate. n > table length -> 'table too short'. A = sum_{k<n} v^(k+1) kp q_k with exact rationals; immediate payment multiplies benefit*A by i/ln(1+i). Return the premium in cents rounded half-up.
