Case contract

A 4:2:0 toy image has a byte luma plane plus independent U and V planes with distinct row guards. Chroma rounds odd dimensions up; plane headers are [1,2,3] bytes. Return physical sample locations for in-bounds luma coordinate. Return the named intermediate layout descriptor and final address fields; all quantities are integer byte offsets unless explicitly stated.
