Irreversible menu command is executed as a hover preview.
ROOT CAUSE
The menu-command-preview model applies `p['previewable']` at its supported operation instead of the stipulated transformation.
VERIFIED REPAIR
Apply `p['previewable'] and p['reversible']` at supported.
Unsuccessful approach: The attempted repair `p['previewable'] or p['reversible']` still violates the supported oracle.
Case contract
Hover previews are provisional document overlays; changing preview restores the base, commit applies the actual command once and cancellation removes only the menu-owned preview.
