ROOT CAUSE
The synthetic-status-separate-from-hop decision uses 'callback_status'; this violates the stipulated controller policy.
VERIFIED REPAIR
At this decision use 'callback_status if synthetic else None', preserving the other controller outputs.
Unsuccessful approach: The partial repair uses "callback_status if action == 'manual' else None" and still fails an explicit boundary or control.
